Consent Form for 16 and under Gamers to play M-Rated Games
Parents will be required to fill out and deliver a consent form for any gamers 16 or under before they can play ANY M-Rated Games! If you need a copy of this form you may download it: here.

Conway Gaming Center Rules and Policies (Effective August 25, 2009)
1.Excessive foul language, inappropriate behavior, and unsportsmanlike conduct will not be tolerated. In respect to all of our players, please keep it clean. After all, with everything we do here, it's just a game. This offense will run the risk of being suspended or BANNED from tournaments and events here at Conway Gaming Center.

2.No alcohol or tobacco products may be visible while inside Conway Gaming Center. Customers who smoke are allowed to do so at least 15 feet from the doorway. At our current location, that is at least at one of the corners of the building.

3.Credit is available at a rate of 50% Credit and 30% Cash on trade-ins of: Magic: the Gathering, Universal Fighting System, World of Warcraft Miniatures, World of Warcraft Trading Card Game, and possible collectible games to be added here in the future. Bulk rates may be available based on our current stock and needs.

4.Credit is for cards and Game Time only. You may get boosters for double credit, but credit earned through events and trade-ins may only be used for cards. Supplies, snacks, and other items are off limits.

5.If you win an event here that awards prizes that are exchangeable for Credit, then you must make an immediate decision. Credit for booster packs will be given at a rate of $2 Credit per pack. Once the credit is applied, you may not change your mind.

6.In any Single or Double Elimination Tournament or Event �¢ï¿½ï¿½splitting prizes�¢ï¿½ï¿½ is not allowed except for between the top two players.

7.�¢ï¿½ï¿½Cutting deals�¢ï¿½ï¿½ to determine winners is not allowed. This offense will run the risk of being suspended or BANNED from tournaments and events here at Conway Gaming Center.

8.Selling anything to another player inside Conway Gaming Center without our consent is not allowed! This includes cards, snacks, running RPG games, etc. This offense will run the risk of being suspended or BANNED from tournaments and events here at Conway Gaming Center.

9.Credit and Time may NOT be transferred between accounts. This includes paying for another player to enter tournaments and events, as well as, trading cards for credit, that is considered selling cards and will not be tolerated.

10.Internet access is allowed here at Conway Gaming Center. With that being said, please be respectful of other players. No adult or offensive website may be visited here. This offense will run the risk of being suspended or BANNED from tournaments and events here at Conway Gaming Center.

11.Conway Gaming Center will be enforcing a strict �¢ï¿½ï¿½you break it, then you bought it�¢ï¿½ï¿½ policy. If you are found to have broken something at Conway Gaming Center, the full price of the item will be deducted from your account in the form of Hours/Credit. Anything remaining unpaid after the deductions will have to be paid in cash.We will not be able to check every item after they are turned back into us (controllers, drum sets, even the Xboxes and PCs themselves). So, make sure and inform an employee if something is broken or not working correctly when you play with it or else if the next player after you informs us that it is broken or not working correctly, we have to look to you!

12.You are not allowed to install any kind of software onto the PCs or Xboxes. This will be considered damaging the PCs and will count towards the �¢ï¿½ï¿½you break it, then you bought it�¢ï¿½ï¿½ policy.

13.Throw away your mess! Conway Gaming Center prides itself on providing a clean and fun environment. Anyone caught littering our premises may have their account deducted in the form of Credit/Hours for our labor to clean up after you.

14.Customers are not allowed in �¢ï¿½ï¿½non-customer�¢ï¿½ï¿½ zones. These zones include: behind the counter, the office, the storage room, or any other place which we deem customers to not be allowed. This offense will run the risk of being suspended or BANNED from tournaments and events here at Conway Gaming Center.

15.Phone calls must be local and should be kept very short. Your account will be charged in the form of Hours/Credit for extended conversations. Any unpaid calls will be paid in cash. Long distance are not allowed. This offense will run the risk of being suspended or BANNED from tournaments and events here at Conway Gaming Center.

16.Animals are not allowed inside Conway Gaming Center.

17.Conway Gaming Center reserves the right to refuse service from any person for any reason.
